

According to our records, you were issued a check by the City of Baltimore between July 1, 2021, and June 30, 2022, that was never cashed. As required by Maryland law, any unclaimed funds must be transferred to the State of Maryland.
Before we make this transfer, we are required to notify you. If you respond and confirm that you have not received or cashed the check, we can issue a replacement payment to your most recent address.
To receive a replacement check, you must complete and return the required form within 30 days of receiving it. You can submit your completed form via:
